1. No Purchase Necessary to Enter or Win
A purchase will not increase your chances of winning.
2. Eligibility
Open only to legal residents of the U.S. Void in Puerto Rico or where prohibited by law. Must be 18 years of age or older to enter or win. A policy purchase is not required to enter. Employees and immediate families (parent, child, spouse, siblings, and respective spouses) of Meadowbrook, Inc. (Sponsor) are not eligible to win. The validity and eligibility of all entries are determined at Meadowbrook, Inc.’s sole and final discretion.
3. To Enter
To enter the 5k Giveaway Contest, (i) visit meadowbrookagency.com/5k-giveaway and complete an official entry form. Limit one entry per person or email address per day. No facsimiles allowed. Sponsor is not responsible for lost, late, stolen, mutilated, damaged, incomplete, illegible, or misdirected entries, or for malfunctions of electronic equipment, computer hardware or software, or any other technical problems related to website entries. Entries become the sole property of the Sponsor and will not be returned. Any attempt at entry except as above stated is void. The Sponsor’s computer will be the official clock of the Giveaway. The 5k Giveaway is subject to all applicable federal, state, and local laws. Any questions regarding the number of entries submitted shall be determined by the Sponsor in its sole discretion, and Sponsor reserves the right to disqualify any entries by persons determined to be tampering with or abusing any aspect of the giveaway or entry process.
4. Contest Timeline
12:00 am EST on April 7, 2023. Log on site and complete an official entry form and send it electronically by 11:59:59 PM EST on May 16, 2023. Entrants are automatically entered into the contest drawing. All entries submitted after the mentioned times/dates are ineligible and will be null and void and not be entered into the 5k Giveaway Contest.
4. Determination of Winners
The grand prize winner will be randomly selected by May 17, 2023. Winners will be strictly required to execute and return the Sponsor’s Affidavit of Eligibility, Liability Release, Publicity Release, and other required legal documentation within seven (7) days of the date of issuance. If such documents are not returned within the specified period, the prize or prize notification is returned as undeliverable, or potential winners are not in strict compliance with these rules, or if the potential winners are unable or unwilling to accept the prize, then the prize will be forfeited and, in Sponsor’s sole and final discretion, and time permitting, an alternate winner selected. All prizes will be awarded. The decision(s) of the Sponsor shall be final.
5. Prize
One - Grand prize winner will be randomly selected. Subject to availability, the winner will be notified by phone, email, or mail. Winners are responsible for all applicable laws pertaining to the acceptance, retention, and/or use of the prizes.
The grand prize winner will be randomly selected from all the entries received during the contest entry period. The winner will receive a $250 gift card. The odds of winning are dependent upon the number of eligible entries received.
Federal, state, and local taxes, and any expenses relating to the acceptance, retention, and/or use of the prize are the responsibility of the winner. Winners may not substitute, assign, or transfer prize, but Sponsor, except where legally prohibited, reserves the right, at its sole and final discretion, to substitute a prize (or portion thereof) with one or more of comparable or greater value. Entrants, by participating, agree that Sponsor, and its respective affiliates, subsidiaries, parent corporations, retailer, local sponsors, and advertising and promotion agencies, and each of its and their respective officers, directors, shareholders, employees, and agents, and any and all Internet servers and access provider(s) (collectively, "Released Parties") will have no liability whatsoever for, and shall be held harmless by Entrants against, any liability, for any injuries, losses or damages of any kind, including death, to persons, or property resulting in whole or in part, directly or indirectly, from the acceptance, possession, misuse or use of the prize or participation in this Giveaway. Winners, by acceptance of the prize, except where legally prohibited, grants permission to and for Sponsor and its designees to use his/her name, address (city and state), photograph, voice and/or other likeness or image, and prize information for advertising, trade, and promotional purposes without further compensation, in all media now known or hereafter discovered, worldwide in perpetuity, without notice, review, or approval whatsoever.
6. Limitation of Liability
Released Parties are not responsible for printing or typographical errors in any Giveaway-related materials; for stolen, lost, late, misdirected, damaged, incomplete, or illegible materials due to technology or entries; or for transactions that are lost, misdirected, or fail to enter into the processing system; or are processed, reported, or transmitted late or incorrectly; or are lost for any reason, including computer, telephonic, paper-transfer, or human error, or electronic, computer, or telephonic malfunction, damage, error, omission, interruption, defect, delay in operation or transmission, or any combination thereof. If, for any reason, this contest is not capable of proceeding as planned, or infection by computer viruses, bugs, tampering, unauthorized intervention, fraud, actions of entrants, technical failures, or any other causes corrupt or affect the administration, security, fairness, integrity, or proper conduct of the Sweepstakes, the Sponsor reserves the right in its sole and final discretion to award the prize from all eligible, non-suspect entries validly received prior to the action requiring such modification or to terminate, modify or suspend the contest. This contest is governed by the laws of the applicable states and is void where prohibited. Resolution of all claims and disputes shall irrevocably be held and adjudicated in the appropriate court or competent venue within Michigan’s jurisdiction, and under the laws of the state of Michigan.
8. Participation
By participating in this Giveaway, entrants agree to:
a. The Official Rules and the decisions of the Sponsor, which shall be in the Sponsor’s sole discretion and final in all respects; and
b. Release, discharge, and hold harmless, the Sponsor and its parent company, subsidiaries, affiliates, advertising, and promotion agencies, and each of their respective officers, directors, shareholders, agents, and employees of the aforementioned entity, the Released Parties, from and against any and all liability, loss, injuries (including death) claims, or damages, of whatever type or nature, arising out of or related in any way to their participation in the Giveaway and the acceptance, use, misuse, or possession of any prize (including any travel or activity related thereto). Upon entering the Sweepstakes each participant agrees to grant, where legal, to the Sponsor and its designees, the use of their names and/ or likeness for future advertising and publicity purposes without compensation and, upon request, will provide written consent to such use in writing.
As a condition of entering the Giveaway, the participant agrees that:
a. Under no circumstances will the participant be permitted to obtain awards for, and the participant hereby forever waives all rights or entitlement to claim, punitive, incidental, consequential, economic, or any other damages, including without limitation, any claim for damages related to personal injury, rights of privacy, or defamation, other than for actual out-of-pocket expenses., even if notice of such damages has been given or received, or such damages were foreseeable.
b. All causes of action arising out of or connected with this Sweepstakes, or any prize awarded, shall be resolved individually, without resort to any form of a class or collective action, or jury trial which is specifically and irrevocably waived; and
c. Any and all claims, judgments, and awards shall be limited to actual out-of-pocket costs incurred but shall in no event include attorney’s fees. The Sponsor reserves the right to modify prize award procedures. Use of information obtained from participants is strictly subject to the Sponsor’s privacy policies and the rules of this Giveaway.
9. Winner's Name
The names of the winners will be available after June 1, 2023.
